Take part in Community Health Needs Assessment

Every three years, Saint Joseph Health System performs its Community Health Needs Assessment (CHNA) to evaluate the overall health status of the communities it serves. The health system recognizes the University of Notre Dame and its faculty and staff as important members of the community. That’s why the system has asked community partners like Notre Dame to share the CHNA with employees.

The survey is an important guide to the system’s outreach strategies, which are especially important now during the pandemic. Completing the survey allows the health system to create health programming identified by community members as needed. Past topics gleaned from the CHNA have included access to healthy food, physical activity and wellness and mental health resources. 

As a bonus to participants, every two weeks, Saint Joseph Health System will have a drawing and one person will win a Fitbit. The survey must be fully completed for a chance to win.

The survey runs through Monday, Nov. 30. You must be over the age of 18 to take part.
