Free voluntary health screenings are available for benefit-eligible faculty, staff, and enrolled spouses

Get a confidential health screening and be better informed about your health. Learn where you stand on such indicators as blood pressure, cholesterol, body mass index, and more.

Earn a $180-$276 premium credit

Benefit-eligible faculty and staff who complete a screening by November 8 will receive an automatic $180 premium credit. Faculty/staff whose eligible spouse completes a screening by November 8 will receive an additional $96 premium credit.

Schedule your health screening early to be entered into weekly raffles

In addition to earning a credit, benefit-eligible staff and faculty who participate in health screenings will be entered into a weekly raffle to win $400. Screening participants are automatically entered into all remaining drawings. The first drawing will occur the week of August 19 and will continue until the last date of screenings. The earlier you complete your screening, the more chances you will have to win.

Choose one of four screening options:

  1. Screenings at the Notre Dame Wellness Center: Make an appointment anytime between July 8 and November 8, 2019. To schedule, call 634-WELL (9355).
  2. Screenings on campus with Memorial: Screenings by Memorial Health & Lifestyle will be offered at various locations on campus beginning August 14. See the schedule below. To schedule an appointment, call askHR at 631‑5900.
  3. Screenings at Beacon Health & Fitness Locations: Make an appointment anytime between July 8 and November 8, 2019, at three convenient locations in South Bend, Granger and Elkhart. Visit Beacon Health and Fitness to find the location addresses. To schedule at the South Bend Center, call 574-647-2650. To schedule at the Granger Center, call 574-647-8460. To schedule at the Elkhart Center, call 574-647-5580.
  4. Screenings from your own provider: Print a screening pamphlet (PDF) and take it to your own provider to obtain your written results. Present your results to the Wellness Center to qualify for rewards. Co-pays or other charges may apply for screenings obtained from other providers.

Frequently Asked Questions

What is a health screening? A screening is a 15-minute checkup with a professional medical provider. The screening measures common biometric indicators such as blood pressure, weight, cholesterol, body mass index, and more. For the most accurate results, fasting for 8 hours prior to the screening is recommended. Immediately following the screening, the provider will provide you a copy of your results on paper and briefly review them with you. The screening is voluntary and you are not required to participate in the screening.

Who is eligible? Free health screenings are available to 1) full-time, benefit-eligible faculty and staff; and 2) their spouses who are enrolled in a Notre Dame medical plan. Eligibility for credits/rewards requires completion of a screening by the deadline.

What are “premium credits”? If you choose to participate in the screening, there is a $180 annual credit (a $15/month credit) toward medical premium deductions for 12 months beginning January 1, and the $96 credit is an additional $8/month.

Who will know my numbers? Nobody but you and your medical provider. The Notre Dame Wellness Center (operated by Premise Health) and Memorial Health & Lifestyle assure the privacy of screening services in accordance with applicable state and federal laws. The University of Notre Dame has no access to personal health information obtained by Premise Health nor Memorial Health & Lifestyle, including information entered into the Wellness Center Patient Portal.
